Heron Terrace resident Fred Frazer, surrounded by those who helped him, holds his nursing-rehab diploma after graduating from the program.

For nine months, Heron Terrace resident Fred Frazer worked with a collaborative team of nursing rehabilitation, physiotherapy and front-line staff members to regain his mobility to the point he could transfer from his wheelchair.

In mid-June, Fred graduated from the Windsor long-term care home’s nursing rehabilitation program.

Fred began working with the nursing rehab team Sept. 21, 2016 after sustaining a fall while on holiday with his family in New Brunswick. After spending time in hospital, Fred returned to Heron Terrace. He needed help with his activities of daily living and required a full-lift transfer from his wheelchair.

Upon entering the program, Fred made a goal for himself: He wanted to be able to stand on his own. Fred has exceeded his goals and is now able to transfer himself with the help of one person with limited assistance. Before beginning the program, Fred was only able to ambulate using a cane.

Fred is also able to assist with his activities of daily living and can self-propel in his wheelchair.

“He is such a determined man who wants to be as independent as he possibly can, and it truly shows with his everyday life,” Nicole Coatsworth, the nursing rehabilitation lead at Heron Terrace, tells S&R Today.

“His family has expressed how happy they are to hear how far their father has come since his fall.”

Nicole credits the home’s front-line staff members for their efforts to help Fred improve his quality of life.

“The staff worked so much with Fred because they knew how important it was to him to not have to rely on a full-lift (transfer),” she says. “And Fred is a very determined man – if you give him a task, he will try to exceed it.”

Nicole notes that Fred is continuing to work with the home’s physiotherapy team improve his transferring and range of motion.
